Your First Month on a GLP-1: What to Expect

The first month is not really about weight loss โ€” the starting dose is not meant to produce much. It is about getting comfortable with the routine, learning how your body responds, and setting up the habits that will still be working in month eight.

Before your package arrives

Twenty minutes of preparation makes the first week noticeably easier. Have these ready:

A scale, used weekly

Same day, same time, after the bathroom, before eating. Weekly โ€” not daily. Daily weight is mostly water and it will make you miserable for no informational gain.

A tape measure

Waist, hips, chest, thigh. Measurements often move when the scale stalls, and month four is when you will be glad you have a starting number.

Photos

Front, side, back. You will not want to take them. Take them anyway โ€” they are the most convincing evidence you will have in month six.

Electrolytes and fibre

You will drink less by habit and eat less overall. Both are worth having on hand from day one.

Easy protein

Greek yoghurt, cottage cheese, eggs, tinned fish, a protein powder you actually like. When appetite drops, convenience decides what you eat.

A sharps container

Needles do not go in the household bin. Most pharmacies will supply one.

Also worth doing: write down your starting weight, your measurements, and one non-scale thing you want back. Climbing stairs without stopping. A specific pair of trousers. Sleeping through the night. That sentence will matter more in month three than any number.

The first injection

Almost everyone is nervous about this, and almost everyone reports afterwards that it was anticlimactic. The needle is very short and very fine โ€” this is a subcutaneous injection into the fat layer just under the skin, not into muscle.

  1. Take the medication out of the fridge a few minutes early. Cold medication stings more.
  2. Wash your hands. Wipe the vial top or pen tip with an alcohol swab.
  3. If drawing from a vial: draw your prescribed dose, hold the syringe needle-up, tap the barrel and push the plunger gently until any air is gone.
  4. Choose a site โ€” abdomen at least two inches from the navel, outer thigh, or back of the upper arm. Wipe with alcohol and let it dry.
  5. Pinch a fold of skin. Insert at 90 degrees, push the plunger steadily, count to five, withdraw.
  6. Sharps container. Note the date. Done.

Week by week

Week 1 โ€” mostly quiet

Most people notice very little on day one or two. Toward the end of the week, portions start feeling like enough sooner. Some mild nausea or burping is common, usually 24โ€“72 hours after the injection. Weight change this week means almost nothing either way.

Week 2 โ€” the first real signal

This is when most people first notice the change described in Why GLP-1 Medication Quiets the Food Noise โ€” the background thinking about food gets quieter. Some see the first pound or two. Constipation is the most common complaint of week two; fibre and water are the fix, and Managing GLP-1 Side Effects covers the rest.

Weeks 3โ€“4 โ€” the routine settles

Injection day stops being an event. Appetite reduction is steady rather than surprising. Typical first-month loss is 2โ€“6 pounds โ€” and if it is less than that, nothing is wrong. The starting dose is an adaptation dose, not a therapeutic one. The real curve starts after your first increase, as GLP-1 Dosing and Titration, Week by Week explains.

Eating in month one

Three rules do most of the work, and all three exist to prevent problems rather than to accelerate results.

  • Protein first, at every meal. Aim for 25โ€“35g. When total intake falls, protein is what protects lean mass โ€” the reasoning is in Protecting Muscle While You Lose Fat.
  • Eat slowly and stop early. Your stomach empties more slowly now, so fullness arrives late. Eating past the signal is the most reliable way to feel unwell.
  • Drink more than feels necessary. Reduced appetite reduces thirst cues too, and mild dehydration is behind a large share of the headaches and fatigue people blame on the medication.

Things that reliably cause trouble early: large portions, very greasy or fried food, alcohol, and carbonated drinks. None are forbidden forever. All are worth avoiding in the 48 hours after an injection. Full detail in Eating Well on a GLP-1.

What is normal, and what is not

Normal in month oneContact your provider
Mild nausea for a day or two after injectingVomiting that stops you keeping fluids down
Feeling full very quicklySevere abdominal pain, especially radiating to the back
Constipation or looser stoolsSigns of dehydration โ€” dizziness, dark urine, no urine
Burping, or a sulphur tasteA lump or swelling in the neck, or trouble swallowing
Some fatigue in week onePersistent vomiting, fever, or pain under the right ribs
A small injection-site bump or rednessA spreading rash, swelling, or difficulty breathing

What to measure at the end of month one

  1. Weight โ€” one reading, same conditions as your first.
  2. Measurements โ€” waist especially, which often moves first.
  3. Photos โ€” same spot, same light.
  4. How your clothes fit, in one sentence.
  5. Energy, sleep, and joint comfort. These frequently change before the scale does, as GLP-1 Benefits Beyond the Scale describes.

โ€œFour pounds in the first month and I was disappointed. My nurse told me to look at the waist measurement instead โ€” two and a half inches. I had been looking at the wrong number the whole time.โ€
