Its catalog spans different treatments, so ask for the exact preparation and dosage form covered by your proposed plan. The inclusive description does not make every treatment, extra service or pharmacy arrangement identical.

It also notes that some longer plans may require payment of the remaining balance unless treatment cannot continue for medical reasons as determined by a provider.
